It includes every fee in the published schedule. Conditional ones are checkboxes because whether they apply depends on your account. Currency conversion and any subscription you pay are not included.
Is my data sent anywhere?

Why is the fee percentage higher on cheap items?
Flat per-order and per-listing fees do not scale down with the price. On a low-value order they can be a larger share than the percentage fees. That is why bundling usually beats selling single cheap items.
